Emergency Appointments:
Available at an additional charge

Cancellation Policy:
A 24-hour cancellation notice is expected or a $50 cancellation fee will be incurred

Financial Agreement:
Payment is expected at the time services are rendered unless other arrangements have been made

Insurance Billing:
In order to bill your insurance we must have the following information with no exceptions:
• Full name as it appears on your insurance card
• Date of birth
• Social security number
• Guarantor’s Name as it appears on primary insurance- holder’s card (Guarantor is the person legally responsible for patient – self, spouse, parent etc.)
• Guarantor’s DOB
• Guarantor’s SS#
• Name, address and phone number of insurance company
